Aquarian Forms

  • Volunteer Waiver and Release:  This form is required to volunteer at the school.  It covers volunteer needs for special events and all classroom/playground activities.  A new signed form will be required every year.

     

    Walking Field Trips Permission Form:  This form will cover all walking fieldtrips for the year.  A new permission form is required every year.

     

     

    Volunteer Driver Form and Requirements:    Please check with your insurance company to confirm you have the required coverage.   Volunteer Driving forms MUST be turned in, at least 1 day in advance.  The front office needs time to verify the applications.  You may not be permitted to drive if we do not have time to complete the process.  Please bring in the signed driver form along with ALL required documents.
